The Exposed Cerebellum is an item in Risk of Rain 2.

First, you need to reach the Solution Residence and defeat Solus Wing. To reach Solus Wing, you must find and activate the Access Node on the third level of your playthrough. Once you reach the Guide's Gorge, save at least one additional Sentry Key for further actions.

After defeating Solus Wing, tou will receive an item called Encrypted Cerebellum. Make sure to pick it up, then proceed trough the portal in the boss's eye. You will find yourself in a kind of bazaar, and on the left side, you will find the CHEF managing a small kitchen. Interact with him, and he will offer to combine items. Give him the Encrypted Cerebellum and any of the following items:

1. Sentry Key

2. Prison Matrix

3. Empathy Cores

4. Encrusted Key

The CHEF will combine the items and give you Exposed Cerebellum. Take this item and give it to large solus unit near the kitchen. The unit will open the Decoded Portal, which will lead to a battle with Solus Heart.
